Patient Information


We see patients on an appointment basis only, please call to schedule and if you need a referral please obtain this from your primary care provider. New patients should arrive 15 minutes early and we request that you bring an accurate list of medications, insurance card, pharmacy information, and any pertinent medical records. Please provide at least 24 hours notice for any cancellations as a courtesy to other patients that may be able to utilize that appointment time.

Illness after hours


Please call the office number during office hours and after hours you can leave a message for urgent matters with the answering service. No prescription refills will be provided outside of regular office hours, and please allow 3 business days for local prescription refills and 10 business days for mail order prescriptions.
