Patient Information



Office Guidelines
Children with diabetes are to be seen every 3 months, once stabilized.
Children with thyroid disease should usually be seen every 2 - 6 months.
Children on growth hormone replacement should usually be seen every 3-4 months.
Children with precocious puberty on therapy should be seen by the endocrinologist every 4 months.
If on lupron, children should be scheduled for their monthly injections as a nurse visit every month. These appointments are scheduled at 15 minute intervals.

What to bring to the office visit if your child has diabetes
• Bring your meter and logbook
• School or camp forms, please complete all information as appropriate
• Come with a list of your medications and needed prescriptions
• Write down any questions prior to your appointment

Cancellations
Please contact the office 24 hours before your child's appointment if you need to reschedule your child's appointment. We will attempt reschedule your child's appointment at the next available timeframe. It is important to maintain regularly scheduled appointments for your child

Rx Refills
Please have your pharmacy fax prescriptions to the office for refill. Refill requests made to MD on call will be filled for a single dose only. You will be required to have your pharmacy send in a fax request for your child's monthly refill.

Illness after hours


Sick day management
A sick day plan will be provided for you at the time of your first office visit. It is always advised to contact the office early in the course of the illness. When calling the office please have your child's current medication list and dose, and pharmacy phone number. If your child has diabetes please know your child's current blood sugar, ketone level and most recent insulin dose prior to call. Please know that depending upon your child's symptoms you may be advised to take your child to the emergency room.

Emergency calls
• Always call the office if your child has experienced a seizure from a severe low blood sugar.
• Call for vomiting more than 2 times within 24 hours
• Positive ketones
• Use of internet e-mail communication
