NCQA's Patient-Centered Medical Home 2011 is an innovative program for improving primary care by facilitating partnerships between individual patients and their personal physicians and when appropriate, the patient's family. With the assistance of both Adventist Health Network and the Adventist Health Partners Quality Team, the following Primary Care Division Practices have received recognition by NCQA for Patient-Centered Medical Home at the highest level, Level 3:

Our role as your health care team in a Medical Home:
  • Provide you with more personalized care because your physician and office team knows you and your health history
  • Provide a safe envrionment to talk about confidential concerns
  • Provide responses to your questions and concerns
  • Provide an explanation of services offered within the practice, for example, why we are taking blood today?
  • Guide you through the complex health care system to help you get the care you need from our office, another physician office, hospital, or other services in the health care system
  • Coordinate your care with you and your family within our office, among specialists, hospital, and other health care services

Your role as a patient in a Medical Home:
  • Be an active participant with your health care team. Call and ask questions, bring a list of concerns and questions to discuss with doctor, nurse or office staff
  • Provide us with a list of all medications, including supplements and treatments you buy in the store or obtain from others so we can help prevent interactions
  • Call us before you go to the emergency room or let us know as soon as possible that you have been to the emergency room, or urgent care
  • When you go for care elsewhere, (for example, drugstore for flu shot), make sure to tell them who your doctor is and authorize them to send the information to us
  • Be informed about and involved in your healthcare decisions. Ask us for information about your condition or treatment
